Participation in this program requires your agreement to the following conditions. Failure to comply with these conditions for the duration of the program may result in your removal.

I will be open and receptive to regular communication with my mentor.

I will comply with program facilitators when solicited for progress reports/reporting.

I will engage with modules every month and complete the post-module surveys.

I will prepare questions and discuss the content from the monthly modules with a mentor.

I will present an action plan to the mentor, that will regulate the interactions between the two parties, prior to our first meeting (which will be the topic of discussion).
